If your child has an allergy or intolerance to food served in our cafeterias and an alternate lunch is required, this form needs to be completed by your child’s doctor and returned to our Food and Nutrition Services Director, Caroline Trinder at Old Orchard Beach High School. Please note that the only substitutes allowed by the USDA for milk are Lactaid and Soy Milk and must be specified by your child’s doctor. Any other milk substitute will need to be provided by the parent or guardian.
